PHYSICAL ACTIVITY READINESS QUESTIONNAIRE (Medical History)

  • Has a doctor ever said you have heart trouble or a heart condition?
  • Has a doctor ever told you that you are diabetic?
  • Do you frequently have pains in your heart or chest?

FIVE COMPONENTS OF FITNESS (PARTIAL LIST)

COMPONENT #1: GOAL SETTING

  1. What are your fitness goals?
  2. By when would you like to accomplish your fitness goals?

COMPONENT #2: NUTRITION/SUPPLEMENTATION/HYDRATION

  1. On a scale of 1-10 (10 being excellent), how would you rate your eating habits?
  2. How many times per day do you eat, including snacks?

COMPONENT #3: CARDIOVASCULAR HEALTH

  1. Are you currently involved in a cardiovascular program?
  2. Do you know your Resting Heart Rate?

COMPONENT #4: RESISTANCE TRAINING

  1. Are you currently involved in a resistance training program?
  2. Do you know multiple (2-3) exercises for each muscle group?

COMPONENT #5: EDUCATION AND PROFESSIONAL ASSISTANCE

  1. Do you have a workout partner?
  2. Do you have a support system as you begin your fitness program?
